Pre-registering your interest
This is currently a page for pre-registering your interest in accessing your diabetes-related medical records.
We anticipate this part of the site (which we call ‘patient access’) will be available from late April 2018 onwards,
at which point we will contact all pre-registrants to help them fully enrol onto the site.
The following section helps explain the patient access site and is provided for information only at this stage.
My Diabetes My Way allows you to access your diabetes information securely. The information is collected from:
- Your GP Computer record
- Your hospital clinic computer record
- Other computer systems relevant to diabetes
Pre-registration (or simply ‘registration’ once the patient access site is Live) includes your name,
address, email address, date of birth and your NHS number, if you know it.
We will use these details to verify your identity and to find your records on NHS systems.
We also use your details to send you our enrolment form and your user account login details.
Your information is sent to us securely and we will not share it with any unauthorised third-parties.
Please note that this service is only currently available to people with diabetes registered at a GP Surgery in Somerset.
